The designation "monsieur"

I'd like to check whether a certain statement is true.

A 19th Century author stated that in late medieval France the
designation "monsieur" was exclusively used for: (1) princes of the
blood, related to the royal family (however distantly); (2) someone
holding a high office of the crown; (3) high level churchmen.

I suspect this principle was not as rigid as the author made it, but
I'd like to have your opinions one way or another. Thanks.

Off the top of my head, I'd think that 'monseigneur' is what the
author meant; I've seen it used in those ways. 'Monsieur', AFAIK, is a
lower social level than that.
