Evidence of death date for Bertha of Holland

Looking for a primary source (likely a chronicle) for the death date
of Bertha of Holland (c.1055-1094) was the first wife and queen of
Philippe I, King of France. The date often given is 1094.

This year is given in the chronicle of St-Pierre-le-Vif at Sens, formerly
ascribed to Clarius - "Anno M XC IIII....obiit Berta regina, que a rege
Philippo prius fuerat derelicta" (1094...Queen Berta died, who had been
repudiated beforehand by King Philip). She retired to Montruil after her
separation from her husband. Different obituary notices place her death on
30 July or 15 October. Patrick van Kerrebrouck only gives the former,
without qualificaton. However the latter date is more plausible, because an
excommunication of the king was renewed on 16 October (when news from July
would have reached the Council of Autun, where this was done) making no
mention of her death in recital of the cause, that Philip had taken another
wife while she was living ("vivente uxore sua Berta").
