In any case, I agree that using a pre-printed form will get you better
results--although I predict that asking just a few specific questions at a
time would be even more successful!

Might I suggest you make things easier for yourself by using the
Family Group Record form from Ancestry?

If you must provide instructions - and I'm sure you will, human
nature being what it is - the pre-printed form will reduce or
eliminate a lot of confusion.

Personally, "When and Where were you born" is a more conversational way
of asking and not nearly so intimidating. What I've found by sad sad
experience is that if I ask questions they don't have the answers to
they don't send the form back.
