Hi All

Is anyone else researching the family of Pringle and Anna SHAW formerly of
Ontario and by 1920 census at Molalla?

Pringle died 1924 registered at Portland and Anna 1932 Clackamas. If I
ordered death certificates what information might I get from them?

I think that each state is a bit different in the format of the death
certificate and there are also variations over the years so I can't say for
sure what you will receive. I can tell you that I requested a death
certificate on my ggggrandfather who died in 1899 in Maryland. His death
certificate was actually two pages, the first was a form with his full name,
place of death, date of death, age at death (I had to calculate his date of
birth), place of birth, nativity (i.e. US born), occupation, sex, marital
status, race, spouse's name, names of his parents, cause of death, doctor's
name and address. The second page was just a hand-written note of
explanation as to the circumstances leading up to his death, written by his
doctor but the account was given my his wife.

Also, on a later death certificate for my ggggrandmother (also from
Maryland), they included information on where she was buried and the
informant was her daughter (with her married name which was helpful). I hope
that gives you some idea what you might expect to receive on a death
certificate.

I have 1922, 1927 and 1935 Oregon death certificates from Multnomah and
Union counties. There is a slight difference in the layout of the page
between the counties, but the info is nearly identical. Info included is:
1. Place of Birth - County, state, township/village/city.
2. Full name and address.
3. Sex
4. Color or race.
5. Marital status and name of spouse or former spouse.
6. Date of birth
7. Age (years, months, days)
8. Occupation and name of employer
9. Birthplace
10. Name of parents and birthplaces.
11. Informant and date filed.
12. Date of death.
13. How long the doctor had attended to the deceased.
14. Cause of death and contributory causes.
15. Other medical info; where disease was contracted, any operations,
autopsy performed, diagnostic tests, etc. and doctor's signature.
16. Place of burial and the undertaker.

Of course, they felt free to leave any of those spaces blank if the
informant didn't know the info, so the usefulness can vary widely.

Next questions are I have a reference number - how do I order a certificate
and what does it cost? Can it be done online and do they take credit cards?
