Missing relative North Bay, Ontario, Canada

I got some help and found out that my great gandfather died at Queen
Victoria Memorial Hospital, North Bay, Ontario, in 1909. I'm trying to find
out what happend to him after he died. I have been told that as a foreigner
(Norwegian), everything he owned was consficated by the Canadian Government.

I have his certificate of death.I have tried the cemeteries in the area
without luck. I was wondering if anyone knows how I can get further. Could
the pasient records still be kept somewhere, does the hospital still exist,
do the Govenment have any records?

Any help would be apreciated.

Well, that hospital is not listed in the online telephone directory.

Perhaps you might try writing to the Nipissing Branch of the Ontario
Genealogical Society. Here is their web site, and there is an email address
at the bottom of the page.

Medical records can be obtained by using the Freedom of
Information forms and submitting this to the correct
government agency.

Medical Records in Ontario are held in various places -
some are at the Ontario Archives, others are with the
original institution. To obtain these records, you should
begin with the OA. Find out what they have, and then how to
request it.

I was able to obtain over 100 pages of records from the
Ontario Archives for a patient in London Ontario in the
1920s, after I submitted an official request complete with
Freedom of Information Act request form. Some of the more
sensitive material in the case logs was blackened out by
censors, but most of the file was intact. The cost was
minimal and was well worth it.

The first thing you must do is find out if those records
still remain at the Hospital, or if they've been moved to
the Ontario Archives.

If you don't see the North Bay institution there, write or
phone OA. Give them the name of your great grandfather, the
dates he was in the hospital and the hospital name. They
will check to see if they hold those files.

their address, phone # and email. They are extremely good
at responding to email enquiries IF you are concise and
provide details they need to give you an answer.

You will find a Freedom of Information request form at
http://naturalizationrecords.com/canada/ It is called
"Access to Information Request Form" click on the link
provided to get the form.

You MUST submit your request plus $5.00 Canadian to the
correct government institution, so be sure you first find
out who holds those records.
