How can I convert a form designed and written in MSWORD (or in LOTUS
WORDPRO) into a gedcom format readable by genealogical software?

Any help would be gratefully received.

I think this is difficult, but have a look at

which describes the "technical part"

I tried a search at Google, but not much to find.

The whole purpose and idea of a GEDCOM file is that there are no forms, no
templates,
it is just a text file written in such a way that any genealogy program
could recognize it
and imput the data. The genealogy programs I am familiar with can not read
a form
created by some other program.

How did you generate the form? Does it come from a genealogy
application? If so, export your data as a GEDCOM diretly from the
original application.

If you created it manually, then your best bet (and it's very tedious!)
is to type the data into e.g. Personal Ancestral File (which is a free
download from <http://www.familysearch.org>) and then export it as a
GEDCOM.

To attempt to create a GEDCOM file manually is just asking for trouble.
